Through the Mayo Clinic & Illinois Alliance for Technology-Based Healthcare, the University of Illinois Urbana-Champaign offers a campus pre-selection process for the Mayo Clinic Summer Undergraduate Research Fellowship (SURF). Illinois sophomores and juniors with a GPA of 3.0 or higher are eligible to apply. Preference is given to students seriously considering careers in biomedical research, particularly through PhD or MD/PhD training. The pre-selection process seeks to support future biomedical innovators who will approach their research through a health equity lens.
Participation in Illinois pre-selection is optional. Students may apply directly to Mayo Clinic. Students selected through Illinois pre-selection must still submit the required Mayo Clinic SURF application.
This form is the first step in the Illinois pre-selection process. You will provide your contact information and the names and email addresses of two recommenders. When you submit this form, each recommender will receive a unique link for uploading a confidential letter, and you will proceed to the full Illinois application. Please verify the email addresses and notify your recommenders before submitting their information.
The completed Illinois application and both letters of recommendation are due by Sunday, Nov. 1 at 11:59 p.m. CT.
